Annuity Fund

Supplemental Unemployment Benefits (SUB) 
‍The Local 28 SUB Fund provides financial assistance to eligible members during periods of unemployment. To start a new claim, contact the Funds Office for an initial application form. Completed forms and proof of State Unemployment payment can be faxed to: (516) 742-8475.

Dental Benefits
‍Local 28’s Dental Plan is administered through the Sele-Dent provider network, offering members access to quality dental care at reduced costs. If you choose to go out of network, reimbursement will be limited to the in-network rate, and you’ll be responsible for any remaining balance.

Optical Benefits 
‍Local 28’s Optical Benefits program provides coverage for eye exams and eyewear through participating providers. Members can access services through CPS, Vision Screening, or Davis Vision.

How to Use Your Benefits:

• CPS / Vision Screening: Call the Funds Office at (516) 742-9478 to obtain a voucher before scheduling your appointment.
• Davis Vision: After making an appointment with a Davis Vision provider, contact the Funds Office at (516) 742-9478 for authorization prior to your visit.
  Out-of-Network Providers: Reimbursement is available up to $150.

Scholarship Opportunities 
‍Local 28 is proud to support the educational goals of our members’ families through two scholarship programs:

• Local 28 Scholarship Plan – Each year, Local 28 hosts a competitive exam for dependents and grandchildren of members, offering scholarships for two- and four-year colleges or universities. Applications are mailed to members and available for download through Connect Plus+. For more information, call the Funds Office at (516) 742-9478.
• SMART Scholarship Plan – The International Association (SMART) also offers scholarships to members. Applications and detailed information are available online, and members can download a Request for Application and Informational Packet through Connect Plus+ .

SASMI Benefits 
‍SASMI provides valuable programs to help members prioritize their health and wellness. One key offering, the Annual Physical Benefit, rewards members and their families for staying up to date with preventive care — members can receive $200 per year for their own physical exam and $100 per covered family member (up to $600 per family, per year).

To qualify, you must be an active member in good standing and eligible at the time the preventive care is received and when applying for the benefit.
