How to Use Your
Optical Benefits: For CPS and Vision Screening You must call the Funds office at (516) 742-9478 to acquire a voucher before making an appointment with a CPS or Vision Screening Provider. For Davis Vision After making an appointment with a Davis Vision provider, you must contact the Funds office at (516) 742-9478 so an authorization can be given before services can be performed. If you use a provider out of network, you will be reimbursed up to a maximum of $150. (For a vision application form, click HERE) |
