​For CPS and Vision Screening
You must call the Funds office, (516) 742-9478, to acquire a voucher before making an appointment with a CPS or Vision Screening Provider.

For Davis Vision
After making an appointment with a Davis Vision provider, you must contact the Funds office so an authorization can be given before services can be performed.
​
If you use a provider out of network, you will be reimbursed up to a maximum of $100.
