Important Letter to members on the use of In-Network vs. Out of Network Providers.
The Welfare Fund’s Preferred Provider Organization (PPO) medical and hospital network is Empire BlueCross BlueShield. If you would like to browse the listing of providers participating in the Empire Blue Cross BlueShield network, you can do so by going to www.anthem.com
Sele-dent is the provider network used by the plan. If you go out of network, you will only be reimbursed the network amount and you will be responsible for any differences. To find a participating dentist, visit www.Sele-dent.com or call 1 800 520 3368
For CPS and Vision Screening - You must call the Funds office, (516) 742-9478, to acquire a voucher before making an appointment with a CPS or Vision Screening Provider.
For Davis Vision - After making an appointment with a Davis Vision provider, you must contact the Funds office so an authorization can be given before services can be performed.
If you use a provider out of network, you will be reimbursed up to a maximum of $100.00

For a new claim, call the Funds office for an initial application form. A completed form along with proof of payment from State Unemployment may be faxed to the Funds office. Fax: 516 742 8475
